Death after TAVR: New data support use of better patient assessments

A large new analysis reveals that most deaths occurring within the first year after transcatheter aortic valve replacement, or TAVR, are not caused by heart problems. The findings suggest that care teams may need to look beyond cardiac function when deciding if a patient is a good candidate for the procedure.

Researchers examined data from thousands of patients who underwent TAVR and found that non-cardiac issues were responsible for the majority of fatalities within 12 months. Common causes included infections, cancer, respiratory failure, and general frailty. This challenges the assumption that a successful valve implant alone guarantees long-term survival.

The study highlights a gap in current pre-procedure evaluations. While doctors routinely assess the heart's structure and function, they often spend less time evaluating a patient's overall physical reserve, cognitive status, or risk for falls. The data suggests that a frail patient with multiple chronic conditions may not see the same benefit from TAVR as a healthier individual, even if their heart valve disease is equally severe.

Experts are now calling for more comprehensive geriatric assessments before the procedure. These would include tests for mobility, nutrition, and mental health. The goal is not to deny TAVR to older or sicker patients, but to better inform them of the real risks and to address underlying health issues before and after the operation. The message is clear: a successful valve replacement is only one piece of the puzzle.
