Clopidogrel monotherapy bests aspirin when patients with diabetes undergo PCI

A fresh look at data from the SMART-CHOICE 3 trial suggests that diabetic patients who receive a coronary stent may do better on clopidogrel alone than on aspirin alone, once the standard initial dual antiplatelet therapy (DAPT) period is over. The findings point to a meaningful shift in how maintenance therapy could be managed in this high-risk group.

The analysis focused on patients who had completed three to six months of DAPT, which typically combines aspirin with a P2Y12 inhibitor like clopidogrel. After that window, the question has always been which single drug offers the best balance of protection against blood clots and risk of bleeding. For people with diabetes, the stakes are higher because they tend to have more aggressive artery disease and a higher chance of stent-related complications.

According to the results, clopidogrel monotherapy led to fewer major adverse cardiovascular events, including heart attacks and strokes, compared with aspirin alone. The difference was driven largely by a reduction in ischemic events, while major bleeding rates were similar between the two groups. This suggests that clopidogrel provides stronger antiplatelet effect without adding extra bleeding risk in these patients.

The researchers note that diabetes alters platelet function in ways that may make aspirin less effective. Clopidogrel, by blocking a different pathway, appears to offer more consistent protection. While the study is a post-hoc analysis rather than a prospective head-to-head trial, the data adds weight to the idea that clopidogrel should be the preferred single agent after DAPT in diabetic patients undergoing PCI.

Doctors often default to aspirin because it is cheap and familiar, but these results challenge that habit. For now, the decision should be individualized, but the evidence is leaning clearly in one direction. Larger randomized trials are still needed to confirm the findings, but the clinical signal is strong enough that many cardiologists may start reconsidering their standard practice.
